Will this show up anywhere my department can see?
Your record here is a confidential medical record held by our practice, protected by HIPAA and Washington law. We release information only with your written permission or in the narrow situations the law requires.
When could you share my information without asking?
Only in the narrow situations the law requires. If we believe someone is in real danger, the law expects us to act. We're required to report suspected abuse of a child or a vulnerable adult. And a judge can order records released. That's the honest list. Ask us about any of it before you book and we'll give you a straight answer.
What about fitness for duty concerns?
Getting treatment on your own is different from a fitness for duty evaluation, and seeking care voluntarily is generally seen as a strength. If you have concerns about your agency, license, or clearance, bring them to the first visit. We'll talk it through before anything goes in the plan.
I'm not in crisis. Is this still for me?
Especially then. Most of what we treat builds quietly over years of calls. Sleep that never fully recovers, a shorter fuse at home, feeling flat off shift. Treating it early is faster and easier than waiting for the wheels to come off. Does the rate apply to my family?
Yes, through our Family Rate. Every additional member of your household gets the same numbers you do: $250 evaluations and $155 follow ups.
